Portages Digital Therapeutic

The Portages Digital Therapeutic is our proprietary science-based, whole-person, habit and behavior change program that is precisely personalized to each patient. Patients use Portages to transform their health behaviors and habits, build resiliency, and overcome stress, anxiety, depression, and substance abuse. Portages can be used in a self-guided manner, and with provider support and guidance. Our providers have proven that using the Portages Digital Therapeutic with patients significantly reduces symptoms AND leads to actual positive change in skills and behaviors in only a few months of programming.

The patient routinely completes a brief assessment of their current mental health and wellbeing status that precisely personalizes programming.

Nudges

Using habit formation techniques, patients receive personalized nudges within the app to prompt them to engage in specific strategies throughout the day. Patients are also able to send and receive nudge messages from their linked companions and receive nudge messages from their linked health care providers and Portages practitioners.

Portages is Demonstrated to Effectively Change Habits and Behaviors and Improve Depression and Anxiety

In our Proof of Concept study of 125 adolescent and adult patients referred for mental health care by their PCP, patients were engaged in using the prototype online version of the Portages Digital Therapeutic with their Therapist or Psychiatry Provider over an average period of 3 to 6 months. All patients in the study completed at least 4 Self-Assessments and received at least 6 Portages.

  • 74% improved their health practices in the Internal World, Physical World, External World, or Spiritual World, where each World is made up of 3 Health Practices and is a measure of actual behavior change.

  • 89% reduced perceived stress, anxiety, or depression scores on standardized measurements

Our three integrated apps for Self Care, Primary Care, and Mental Health Care facilitate rapid and seamless access to care, unite patients and providers, guides care with precision powered by our Portages Digital Therapeutic, and  automates practice management including care navigation and documentation.

Self Care App

Portages guides Patients through self-assessments and personalized self-care programs and links them with their primary care providers, care managers, Portages Health providers, and their personal companions for guidance and support.

Primary Care App

Portages guides Primary Care Providers and Care Managers to monitor patient progress, improve engagement, and collaborate with Portages Health Practitioners.

Mental Health Care App

Portages Digital Clinics

Through the Portages Digital Clinics, expert teams of therapists, psychiatrists, coaches, and care navigators provide a range of specialized, time-limited programs for mental health or substance abuse using digital and telehealth technology. A full continuum of services that vary in intensity are matched to each patient’s current needs.  With the Portages Way, the Portages Digital Therapeutic, and the Portages Collaborative Technology, we provide precisely personalized evidenced-based care delivered with scientific rigor. Our care is more efficient, more effective, and achieves significant cost savings when compared to “Care as Usual”.

“Care as Usual” often involves poor access to care, poor quality, and poor results.  As a result, care as usual is costlier leading to preventable Emergency Room visits, hospitalizations, and high number of encounters.

Patients with both substance abuse and depression (Dual Disorder) who have the highest acuity and complexity are classified as “Very High Risk”.  These individuals may typically have multiple ED visits, at least one psychiatric hospitalization and at least one admission for medical detox in a year. Their annual treatment costs (not including pharmaceutical costs) can be over $50,000.  The financial burden to payors AND patients is extraordinary.

Those same individuals receiving care through Portages Health over the course of a year may undergo 2 Intensive Outpatient Program episodes of care, 2 Group Outpatient episodes of care, and at least one Coaching episode of care while receiving individual therapy and psychiatric care and avoiding ED visits, hospitalizations, and detox admissions. The annual cost to payors would be on average only $14,500 , leading to a savings of 365% when compared to costs for “care as usual”.  Patients at a moderate risk for depression, anxiety or substance abuse often undergo longer term psychotherapy because no other options are available to them. These same patients could benefit from the Portages Health Coaching Program and experience improved efficiency and effectiveness of care at a fraction of the cost.

Portages Health is able to provide care more efficiently and more effectively leading to profound savings when compared to costs associated with care as usual. The table below illustrates the average annual costs and the potential savings to payors comparing care by Portages Health to “Care as Usual” for individuals with Mental Health conditions and Substance Abuse Conditions based on a stratification of level of risk.
